Fraxel Post-Care Instructions

What to Expect:
The intensity and duration of your after effects depends on the treatment aggressiveness and your individual healing characteristics. 3-7 days of pink skin is normal depending on the strength of treatment.
Immediately after the treatment, you will experience redness, swelling, and sometimes pinpoint bleeding. Over the next few days, redness usually worsens. Most of the swelling appears on the first morning after treatment, particularly under the eyes. Swelling usually lasts 2-3 days. To minimize swelling:

  • Ice the treated area for 10-20 minutes every hour. Icing is for healing and swelling prevention as well as soothing, so ice as instructed even if you are not hot or uncomfortable.
  • Apply Biocellulose Restorative Masque between icings, leave on for 20-30 minutes, then continue to ice (ice or frozen peas). You may use multiple masques the first three days for comfort and cooling.
  • If given Arnica, take it under the tongue three tablets under the tongue, three times per day for four days.
  • Use the Alastin kit as instructed.
  • Sleep with your head slightly elevated the first night if possible.
  • Heat sensation can be intense for 2–3 hours after the procedure. Occasionally oozing can occur in isolated areas for a few days.
  • Your skin may appear bronzed or have little dark dots on the treated area. It may feel dry, peel, flake, or have a “sandpaper” texture a few days after treatment.
  • Dead skin and pigment are normal and should start sloughing off 3–4 days after the treatment. This process takes about a week for treatments on the face. (For off-face areas, such as neck, chest, hands/arms, healing is slower and the process may take up to 2 weeks.)
  • Once the sloughing is complete, you may notice some pinkness over the next few weeks. Most redness resolves during the first week after treatment, but a rosy “glow” can remain for several weeks. You can apply makeup to minimize the redness once cleared by Dr. Seiler or his assistant.
  • Benadryl or vinegar soaks can be used to soothe the skin, decrease itching and help with sloughing and shedding of skin and pigment. Vinegar soaks (2-3 ounces of white vinegar in iced water bowl, soaking wash clothes and applying to skin without rubbing/scrubbing) should be performed 3-4 times per day or more often if needed.
  • If you have a flare-up of acne or formations of milia (tiny white bumps on the skin) contact our office, and we can discuss management of the break out. Note that some breakout is normal in the healing process.

Immediately After Treatment

  • Ice 10-20 minutes every hour for the rest of the day or more often desired.
  • Apply Biocellulose Restorative Masque between icings, leave on for 20-30 minutes, then continue to ice (ice or frozen peas). You may use multiple masques the first day or two for comfort and cooling.
  • Apply CICALFATE to decrease redness and moisturize. Apply as needed throughout the day (at least every 1-3 hours).
  • The night of treatment, you may shower but avoid excessively hot water.
  • Use the ALASTIN GENTLE CLEANSER to wash the skin.
  • Apply ALASTIN MOISTURIZER for cooling and moisturizing.
  • Advil (ibuprofen) 600mg three times per day or Aleve 500 mg two times per day may be taken as an anti-inflammatory to help decrease discomfort and swelling.

Post-op Day 1 (Day after treatment):


Morning Product Regimen:

  • Wash skin with ALASTIN GENTLE CLEANSER. This cleanser is VERY CONCENTRATED so you only need a small amount (less than the size of a pea). If you have not used this cleanser before, pay close attention to the amount of product that you are using because using too much product can make it more difficult to remove.
  • Apply ALASTIN SKIN NECTAR (1 pump for face or 2 pumps for face, neck and chest).
  • Apply ALASTIN MOISTURIZER.
  • Apply CICALFATE as needed for redness, itching and additional moisture.
  • Apply ALASTIN SUNSCREEN SPF30+ if traveling in the car or going outdoors for any reason.

Throughout the Day:

  • Ice 3-4 times per day for about 10-20 minutes.
  • You may also use the Biocellulose Restorative Masque as desired.
  • Reapply ALASTIN MOISTURIZER as needed to keep the hydrated.
  • Reapply CICALFATE as needed for redness, itching and additional moisture.
  • Begin vinegar soaks to help with itching/irritation and sloughing. Put 2 ounces of vinegar in a bowl of cold water; soak wash cloths and apply to treated area for 10-20 minutes. DO NOT SCRUB, RUB, or USE EXFOLIANTS OR CLARISONIC. Vinegar soaks 3-4 times per day or more often if needed.

Evening Product Regimen:

  • Wash skin with ALASTIN GENTLE CLEANSER.
  • Apply ALASTIN SKIN NECTAR (1 pump for face or 2 pumps for face, neck and chest).
  • Apply ALASTIN MOISTURIZER.
  • Apply CICALFATE as needed for redness, itching and additional moisture.


Post-op Day 2:


Morning Product Regimen:

  • Wash skin with ALASTIN GENTLE CLEANSER.
  • Apply ALASTIN SKIN NECTAR (1 pump for face or 2 pumps for face, neck and chest).
  • Apply ALASTIN MOISTURIZER.
  • Apply CICALFATE as needed for redness, itching and additional moisture.
  • Apply ALASTIN SUNSCREEN SPF30+ if traveling in the car or going outdoors for any reason.

Throughout the Day:

  • Ice if you still feel heat or swelling.
  • You may also use the Biocellulose Restorative Masque as desired.
  • Reapply ALASTIN MOISTURIZER as needed to keep the hydrated.
  • Reapply CICALFATE as needed for redness, itching and additional moisture.
  • Vinegar soaks 3-4 times per day or more often if needed.

Evening Product Regimen:

  • Wash skin with ALASTIN GENTLE CLEANSER.
  • Apply ALASTIN SKIN NECTAR (1 pump for face or 2 pumps for face, neck and chest).
  • Apply ALASTIN MOISTURIZER.
  • Apply CICALFATE as needed for redness, itching and additional moisture.


First Week:

  • Follow-up 2-3 days after procedure to discuss continuing skincare regimen.
  • Continue products as recommended in first follow-up visit.
  • Avoid excessive exercise, perspiring, or exposing skin to heat and sun.
  • Do NOT use products that will cause irritation during this time. Do not use abrasive scrubs, toners, or products that contain glycolic acids or Retin A.
  • DO NOT use Clarisonic brush until approved to do so by Dr. Seiler or his staff.
  • Peeling and/or flaking is normal during the healing process. Continue using Alastin products and Cicalfate to keep skin adequately hydrated.
  • Continue vinegar soaks until shedding of skin and pigment is complete.
  • Once the sloughing is complete, you may resume your routine Seiler Skin approved skin care and make-up products as designed by Dr. Seiler or his assistant.
  • Sunscreen: Avoid direct sun exposure for 2-3 weeks (and as instructed by Dr. Seiler or assistant) after the treatment. Apply sunscreen 20 minutes before going outside. Reapply every hour. If short, direct sun exposure is necessary, wear a hat and clothing that covers the treated area. Diligent sunblock use may lower the risk of laser-induced hyperpigmentation (darker color).
  • Use only Seiler Skin approved sunscreens (SkinCeuticals, SkinMedica, Alastin, EltaMD, Revision Intellishade, ColoreScience, Tizo)
  • Skin Lightening Products: Discontinue use of your skin lightening products while your skin is tender. Restart these when instructed to do so by Dr. Seiler or his assistant (generally restarted at 1 week or more).
  • Shaving: Ask Dr. Seiler or his staff prior to resuming (generally restarted at 1 week or more).
  • Waxing: DO NOT wax the treated area until 4 weeks after treatment.
  • EXERCISE: You may exercise after a couple of days, try not to get sweat on the treated areas. Cleanse immediately.
  • Cold Sores: If you have a history of cold sores, Dr. Seiler will prescribe Valtrex.
  • Abnormal Healing. If you notice any blisters, cuts, bruises, crusting/scabs, areas of raw skin, ulcerations, active bleeding, increased discomfort or pain, pigment changes (lighter or darker than usual complexion), or any other problems, please contact Seiler Skin and Dr. Seiler at the office or on his cell immediately. Post-treatment healing varies from patient to patient.
